Clinic check in:
Please plan to arrive at the clinic site between 11:00 am and 12:00 pm on the first day of the clinic. The FJM staff will post signs marking the on-site registration location. Please check the link to the clinic site you are attending for facility names. Information will be updated the week prior to the clinic.
Attendance information:
Be sure to download the health form and bring the completed form with you to the clinic site. Unpaid clinic fees are also due at the time of check in.
Please download the MP3 files of music to bring with you on an MP3 player or to burn onto a CD before you arrive.

Wright State participants must also bring the completed Wright State Release form to clinic check in.
