Clinic check in:
Please plan to arrive at the clinic site between 11:00 am and 12:00 pm on the first day of the clinic. The FJM staff will post signs marking the on-site registration location. Please check the link to the clinic site you are attending for facility names. Information will be updated the week prior to the clinic.


Attendance information:

  • Be sure to download the health form and bring the completed form with you to the clinic site. Unpaid clinic fees are also due at the time of check in.
  • Please download the MP3 files of music to bring with you on an MP3 player or to burn onto a CD before you arrive.
  • Details of what to bring and additional clinic information can be found in the general information document as well as on our FAQ page.
  • Wright State participants must also bring the completed Wright State Release form to clinic check in.
